Completing Necessary Forms
To ensure we provide you with the safest and most appropriate treatment, it is crucial for us to gather details about your patient information, medical history, and dental history.
We encourage you to complete your new patient forms before arriving at our office to maximize the efficiency of your appointment. We will send you the link for these forms. If you are unable to complete them beforehand, please plan to arrive 15 minutes prior to your appointment.
Your Appointment
Upon arrival, our front office team will greet you warmly and check you in. During your brief wait, please help yourself to a complimentary cup of coffee or bottled water in our reception area.
You will then be guided to our private consultation room, where we will take some time to get to know you personally, address any concerns, and answer your questions. One of our assistants will then gather your information and take necessary photos and x-rays.
Next, you will meet one of our doctors, who will discuss your oral health goals and how we can best help you achieve them. Our doctors will evaluate your needs and develop a personalized treatment plan tailored to your priorities.
After your consultation with the doctor, a knowledgeable team member will review the financial aspects of your treatment, including insurance and payment options, and will schedule any future appointments as needed.
